All referrals must be made by a Physician. Referrals can be sent by confidential fax or email. Referral letters must include a brief description of the current problem, relevant history, and current medication list.

Dr. Tucker will contact patients and request that they complete an intake questionnaire prior to the first appointment.

​

A consultation letter will be sent to the referring physician after the initial appointment, and a discharge letter will be sent at the conclusion of therapy.
